Payment will be due at the time of service, and a detailed receipt can be made available to you, upon request, for reimbursement from your insurance company.
Packages never expire, but prices are subject to change with ample notice provided.
Reimbursement for packages from insurance companies is prohibited by the insurance companies prior to their full use. Beneficially, they, along with single visits, can be purchased with FSA and HSA cards.
